What belongs in your intake form?
Include pet parent and emergency contacts, pet identification, feeding preferences, owner-provided health and medication instructions, behavior and routines, service dates and communication preferences. Home visits can include access arrangements; boarding and daycare can include drop-off and pickup details.
Choose short answers, longer writing areas, dates, checkboxes or single-choice questions. Add your own questions and remove sections that do not apply. Ask owners to share access codes privately, and collect existing medication instructions rather than offering treatment advice.
